Baekrokdam Korean Medicine Clinic · Acne Treatment Guide

If acne subsides while using medication but returns to the same spot after stopping or when exhausted

We see many patients where new comedones and red inflammation return despite repeated extractions, topical medications, antibiotics, or sebum regulators. We determine the priority of Korean herbal medicine prescriptions by analyzing the depth and speed of the acne, heat rising to the face, sebum, digestion, bowel movements, sleep, stress, and the period before and after menstruation.

Rather than a single pimple that appeared today, we look at the overall flow where comedones, new inflammation, and healing marks overlap.

Rather than the number of acne lesions, focus on the depth and sequence of newly formed acne

Take 2 minutes to summarize your skin changes from this week

This is not a tool for diagnosing via photos. Categorize comedones, papules, pustules, nodules, and remaining marks, and connect the location and time when new acne began.

Every morning in front of the mirror

When another breakout appears on the chin where it was calm yesterday

Instead of looking only at a single new pimple, we check if it recurs in the same spot and look for recent changes. Comedones, inflammation, and deep nodules have different treatment priorities, and the start dates of menstruation, medication, cosmetics, or friction can provide clues to the recurrence.

  • Is it acne deep in the skin that hurts when touched, or a surface comedone?
  • Does it recur in the same location, such as the chin, forehead, or chest?
  • Did it start after a new product, medication, supplement, or menstrual change?

From symptoms to conditions

Select the current appearance before the name of the disease

Blackheads, closed comedones, red pustules, deep nodules, and red or brown marks are not handled in the same order. Start based on the shape of the current acne and the speed at which new ones appear.

Comedones · Inflammation · Nodules

Shape and depth of acne

We distinguish the form of acne and the risk of scarring, from blackheads and whiteheads to red inflammation and deep, painful nodules.

Jawline · Forehead · Chest · Back

Recurring Locations and Adult Acne

When acne begins in adulthood or recurs in specific locations, we examine the relationship with hormones, skincare products, medications, and friction.

Discontinuation · Recurrence · Same location

Recurrence and New Inflammation After Treatment

For acne that decreased during treatment but returns after pausing treatment, we examine the sequence of new breakouts along with sebum, heat, sleep, digestion, and recovery conditions.

Erythema · Pigmentation · Atrophic scars · Hypertrophic scars

Red/Brown Marks and Scars

We categorize active inflammation, flat red/brown marks, atrophic (pitted) scars, and hypertrophic (raised) scars as separate conditions to determine the treatment sequence.

Folliculitis · Rosacea · Perioral dermatitis

Other rashes that look like acne

When there are few comedones or when itching, widespread flushing, or small papules around the lip border are primary, we first distinguish between acne and other conditions.

Questions that arise during treatment

How long should I wait, and what should I record?

We address common questions that arise during the treatment process, such as why improvement only occurs while using medication, the order of managing marks versus scars, and the connection between jawline acne and fluctuations in menstruation, digestion, and sleep.

01 · Adult jawline acne

How to combine the management of contact irritation with Korean herbal medicine when jawline acne recurs before menstruation or after overtime work

Jawline acne cannot be explained by hormones alone. We immediately reduce contact irritants such as masks and shaving, and then determine whether the breakouts are flares occurring a few days after overtime work or recurrences returning to the same spot along the menstrual cycle; we then address the internal conditions for recurrence through Korean herbal medicine.

02 · Red bumps along the jawline

Red bumps along the jawline after shaving: How do we distinguish between acne and follicular irritation?

Recurring red bumps along the jawline after shaving cannot be determined as jawline acne or follicular irritation based solely on the time elapsed since shaving. We examine whether comedones are present, whether itchy bumps of similar size appear along the hair follicles, and whether the same pattern repeats outside the shaved area.

03 · The structure of recurrence

When acne returns after stopping medication, what does Korean herbal medicine look at differently?

If inflammation and sebum decreased while using medication, but new acne appears in the same locations after stopping, it is time to analyze the stages of acne and the bodily conditions driving the recurrence, rather than repeating a failed treatment.

04 · Recurrence after extraction

When closed comedones (whiteheads) refill even after extraction

Not all bumpiness felt after extraction is new closed comedones. The next step of treatment changes depending on whether it is a new closed comedone, skin that has become rough due to irritation, or a rash that resembles acne, such as perioral dermatitis or folliculitis.

Medical evaluation focused on changing the interval of recurrence rather than just calming symptoms

We do not just suppress existing inflammation; we examine the conditions of heat, sebum, and circulation that create new acne

Many people find that acne subsides while taking or applying medication, but returns to the same jawline, forehead, or back once stopped. Baekrokdam analyzes the depth of comedones, papules, pustules, and nodules, as well as heat sensations in the face and upper body, sebum, digestion, bowel movements, sleep, stress, and the pre- and post-menstrual periods to determine the proportion of heat-clearing, dampness-regulating, liver-soothing, menstrual-regulating, and yin-nourishing treatments.

Points to note first

If multiple deep, painful nodules appear in a short period, or if swelling and heat spread widely and scarring increases rapidly, please visit a dermatology clinic first. If pregnancy is possible or drug side effects are suspected, do not change medications arbitrarily and coordinate with the prescribing medical staff.

You may consider a consultation in these cases

  • When new acne continues to appear or recurs in the same location despite treatment for the recommended period
  • When the timing of worsening clearly overlaps with changes in menstruation, sleep, stress, or digestion
  • When the skin becomes drier with treatment, but upper-body heat, sebum, digestive/sleep discomfort, and new acne remain

Things to check during the consultation

  • The ratio of comedones, papules, pustules, and nodules in photos, and the speed at which new ones appear
  • Dates of changes in menstrual cycle, sleep, diet, digestion, and acne
  • Topical medications, oral medications, and cosmetics currently in use, and the reactions (improvement or worsening) to them

Deep nodules, widely spreading infections, and rapidly forming scars must be checked at a dermatology clinic without delay. For already pitted scars, separate procedures should be considered; Korean herbal medicine treatment examines the patterns of recurring new inflammation and the state of sleep, digestion, and menstruation that slow down skin recovery.

Medical and content review

We look at the flow of how new acne forms, rather than just a single spot that appeared today

We categorize comedones, red inflammation, and deep nodules, and check if they recurred in the same spot and what remained after treatment. If the risk of scarring is high, we will first explain the priority of dermatological treatment.

“We do not summarize acne with a single sentence about having too much sebum. We ask in order: what has appeared on the skin, when it recurs, and what remained after treatment.”

Frequently Asked Questions

Is acne caused by not washing my face enough?

It cannot be explained by a lack of cleanliness alone. It is a combination of sebum and skin cells clogging pores, bacteria, and inflammation; scrubbing harshly or squeezing can make it worse.

For adult acne, should I visit a dermatology clinic first?

If you have deep and painful nodules, rapid scarring, or sudden severe changes, a dermatology evaluation is the priority. We also check the context of hormones, medications, and products used.

Should I stop using my current topical or oral medications?

Do not stop them arbitrarily. Please bring information on the products used and the duration, as well as any changes in dryness, redness, or new acne. We will determine the goals for Korean herbal medicine treatment by observing your current acne alongside the recurring changes in your overall body condition.

Can scar treatment begin even if acne is still present?

Generally, it is standard to first stabilize active inflammation and the risk of new scars. A dermatology clinic will determine the sequence of treatment by distinguishing between marks and indented or protruding scars.
